What kind of help can I get from a Care Advocate?
Care Advocates can help you find local programs and services, schedule medical appointments, arrange transportation, apply for benefits like SNAP or housing support, and more—based on what matters most to you. Your Care Advocate will communicate with you by phone, text, or email, providing details about resources that meet your needs.
How does Papa Community work?
Once you're enrolled, you'll start with a brief virtual doctor visit to discuss your health history and daily challenges—like transportation, food access, or managing expenses. The doctor will create a personalized care plan, and then we'll match you with a Care Advocate who will contact you to get started.
This initial doctor visit is not for medical emergencies or treatment. If you have a medical emergency, please call 9-1-1. For diagnosis or treatment, contact your primary care physician.
